We recommend that user’s navigate this page with the Internet Explorer browser. However, depending on the client side browser settings, users may have difficulty to download XFA based PDF forms and Excel documents. Below are a series of tips on how to change your browser settings for Chrome. Firefox and Safari:
- Chrome:
- In “Settings”, click on “Show advanced settings…” then scroll to the heading “Downloads”. Check the box associated with “Ask where to save each file before downloading”. Go back to the page and refresh and then download the document.
- Firefox:
- In the horizontal menu at the bottom of the page, click on the “Settings” option. A pop-up box will appear. Under the section “Downloads”, choose the selection “Always ask me where to save files”. Go back to the page and refresh and then download the document.
- Safari:
- Click on the settings icon on the top right hand side of the browser, then click on preferences. Under the section download, ensure that the option “Always prompt before downloading” is ticked.
Note: XFA based PDF forms can only be opened using the latest version of
Adobe Acrobat Reader. Documents in .xlsx format should be opened using Microsoft Excel. Other spreadsheet tools can open these files but we recommend using Microsoft Excel.
Please note that some UIS questionnaires do not display properly in the Chrome browser. Use another browser, such as Internet Explorer or Firefox, or open the PDF with Adobe Reader.

- Dans «Paramètres», cliquez sur « Afficher les paramètres avancés... » puis faites défiler jusqu'à la rubrique « Téléchargements ». Cochez la case associée à « Toujours demander où enregistrer les fichiers... ». Rafraîchi la page puis télécharge le document.
- Firefox:
- Dans le menu horizontal en bas de la page, cliquez sur l'option "Paramètres". Une boîte de pop-up apparaîtra. Dans la section "Téléchargements", choisissez la sélection " Toujours me demander où enregistrer les fichiers ". Retour à la page et rafraîchir puis télécharger le document.
- Safari:
- Cliquez sur l'icône des paramètres sur le côté en haut à droite du navigateur, puis cliquez sur « Préférences ». Dans le cadre du téléchargement de section, vérifiez que l'option « Toujours demander avant de télécharger » est cochée.
Remarque: Les formulaires PDF en fonction XFA ne peuvent être ouverts qu’avec la dernière version
d'Adobe Acrobat Reader . Les documents au format xlsx. devraient être ouverts à l'aide de Microsoft Excel. Autres outils de tableur peuvent ouvrir ces fichiers, mais nous vous recommandons d'utiliser Microsoft Excel.
Veuillez noter que certains questionnaires ne s’affichent pas correctement dans le navigateur Chrome. Comme alternative , vous pouvez utiliser un autre navigateur , comme Internet Explorer ou Firefox , ou ouvrir le fichier PDF avec Adobe Reader .
