The following documents have been prepared for the Fund for the Elimination of Doping in Sport:

National Project Application Form - AD:001 This application form is for national projects only. Applications submitted by an individual State Party should not exceed USD$20,000. In Word format: English \| Français In PDF format: English \| Français Regional Project: Application Form - AD:002A This application form is for sub-regional, inter-regional, or regional projects. The maximum amount of assistance available for these projects is USD$50,000. In Word format: English \| Français In PDF format: English \| Français Regional Project: Letter of Support - AD:002B At least three letters of support should be provided from governments involved in sub-regional, inter-regional, or regional projects. In Word format: English \| Français In PDF format: English \| Français Evaluation Report - AD:005 An evaluation report must be submitted at the conclusion of every project. This report will present the results of the activites financed and the outcomes achieved. In Word format: English \| Français In PDF format: English \| Français Financial Statement - AD:006 An itemized certified financial statement must be submitted at the conclusion of every project. All original supporting documentation, in the form of invoices and reciepts, must be included proving that the funds were used for the implementaiton of the project.
